Intent To Graduate Form

Note: Please fill in all of the boxes. If you do not graduate on the original date that you give us for a graduation date you must RESUBMIT an "Intent to Graduate" form as this form is then invalid.

Attention!

Filing deadline is September 15th for December graduates.
If you graduated last December, you may walk in the upcoming graduation ceremony.
Filing deadline is December 1st for May and August graduates.
You must be able to complete your course work by the end of the summer term in order to walk in the May graduation ceremony.
Any student filing after the deadline is subject to a late filing fee of $50.00
Those filing after February 1, 2010 will NOT be permitted to walk in the May 2010 commencement ceremony.
