3 . Compose a letter of admission addressed to Dr. Patrick Wilson, Dean of the Graduate & Professional Studies Programs. You may send the letter to us by mail to the above address, by fax to 321-3575, or by email to evening@cbu.edu.

4. Print out the Certificate of Immunization and have it completed. You may send the completed form to us by mail to the address above or by fax to 321-3575.
