To apply for a trip, print and complete:

• Travel/Study Abroad application
• Travel/Study Abroad Agreement
• International Trip Student Information form
• Release and Program Conditions
• Flight Responsibility and Outside Travel Waiver
• Health and Safety Questionnaire
• Pay applicable non-refundable fee to Business office to reserve your place. Be sure to indicate trip for which you are paying. Complete payment forms provided by the Business Office when payment is made.
• The Business Office will send payment receipts to Mrs. Anderson.

Passports:

To apply for a passport online you can visit www.passportexpress.com and follow instructions. Passport applications should be mailed about 16 weeks prior to departure or applicant should pay extra fee to expedite. Process is VERY SLOW.
